Powerhouse Elite Cheer & Dance — Participant Waiver & Consent Form

This Waiver and Consent Form applies to participation in the Cheer & Tumble Holiday Camp held on Wednesday 7 October 2026 at 6/42 Turner Rd, Smeaton Grange NSW 2567, operated by Powerhouse Elite Cheer & Dance ("Powerhouse Elite").

1. Liability Release & Assumption of Risk

I acknowledge that participation in cheerleading, tumbling, dance, and related physical activities involves inherent risks including, but not limited to, physical injury, muscle strains, sprains, fractures, and other accidents. I voluntarily accept these risks on behalf of the participant named in this registration. I agree to release, indemnify, and hold harmless Powerhouse Elite Cheer & Dance, its coaches, staff, volunteers, and agents from any and all claims, damages, losses, or liabilities arising from the participant's involvement in this event, except where caused by gross negligence or wilful misconduct. I confirm that the participant is physically fit and able to participate in physical activity. I agree that bookings are non-refundable but may be transferred to another participant with at least 48 hours' notice.

2. Medical Treatment Consent

In the event of illness or injury during the event, I authorise Powerhouse Elite staff to seek and consent to emergency medical treatment for the participant if I cannot be contacted. I understand that any costs incurred for medical treatment are my responsibility. I confirm that all known medical conditions, allergies, and medications have been disclosed in this registration form. I authorise Powerhouse Elite staff to administer basic first aid as required.

3. Photography & Video Consent

I consent to Powerhouse Elite Cheer & Dance photographing and/or filming the participant during this event. I grant Powerhouse Elite a non-exclusive, royalty-free licence to use such images and footage for promotional purposes including, but not limited to, social media, website, and marketing materials. I understand that images will not be sold to third parties. If you do not consent to photography or video, please contact us at admin@powerhouseelite.com.au before the event.
